Background Screening Disclosure and Authorization

1. Disclosure of Intent to Procure a Report

Citovelo may obtain a consumer report (and, in some cases, an investigative consumer report) about you from Checkr, Inc., a consumer reporting agency, for purposes of evaluating your eligibility to serve as a Concierge on the platform. The report may include information regarding your character, general reputation, personal characteristics, mode of living, criminal history, sex offender registry status, identity verification, and, where relevant to the task type, motor vehicle records.

2. Nature and Scope of the Report

• Social Security Number trace and identity verification
• National criminal database search, plus county, state, and federal criminal records
• National and state sex offender registry search
• Terrorist watch list screening
• Motor vehicle records (for tasks involving driving)
• Enhanced screening for senior care tasks, including abuse and neglect registry checks

An investigative consumer report may include personal interviews with persons who know you regarding the categories listed above. You have the right to request a complete and accurate disclosure of the nature and scope of any investigative consumer report, and a written summary of your rights under the Fair Credit Reporting Act (FCRA), by contacting Checkr at support@checkr.com.

3. Authorization

I authorize Citovelo and Checkr, Inc., and their respective agents, to procure a consumer report and/or investigative consumer report about me, and to obtain any additional information necessary to verify my identity and eligibility, including the information I provide during onboarding. I understand that this authorization remains in effect during my time as a Concierge and permits ongoing monitoring and re-screening as described in the Background Screening and Eligibility Policy, unless I revoke it in writing.

I understand that refusal to authorize, or revocation of this authorization, will disqualify me from serving as a Concierge on the Citovelo platform.

4. Information You Provide

To complete this authorization you will provide, through Checkr's secure portal:

• Full legal name and any prior names or aliases
• Date of birth
• Social Security Number
• Current address and prior addresses (typically 7-year history)
• Government-issued identification

Providing your Social Security Number helps ensure accurate matching of records and reduces the risk of false matches. You may decline to provide it, but doing so may prevent completion of the screening and therefore eligibility.

5. Your Rights Under the FCRA — Summary

• You must receive a written disclosure that a report may be obtained (this document)
• You must give written authorization before a report is procured
• If eligibility is denied based on the report, you will receive a pre-adverse action notice, a copy of the report, and a copy of "A Summary of Your Rights Under the Fair Credit Reporting Act"
• You have the right to dispute the accuracy or completeness of any report directly with Checkr
• You may obtain a copy of your report from Checkr under certain circumstances

6. State-Specific Notices

Some states provide additional rights or require specific disclosures. If your state requires a separate disclosure or limits the scope or look-back period of background checks, Checkr will apply those limits during the screening process. For California, Maine, Minnesota, Oklahoma, and other states with specific requirements, additional notices may be presented in the Checkr portal.

7. Data Retention and Security

• Screening data is encrypted in transit and at rest
• Access is limited to authorized personnel with a legitimate need
• Reports are retained as required by law and per Citovelo's retention schedule
• You may request deletion of identity documents after verification; report results are retained separately
